The philtrum is the vertical groove from the base of your nose to the top edge of your upper lip. Adults commonly measure roughly 11 to 15 mm. Philtrum length is the vertical distance from the base of the columella, where the nose meets the lip, down to the top edge of the vermilion border of the upper lip. Adults commonly measure roughly 11 to 15 mm, with men a millimeter or two longer than women on average. Because the whole region is compact, differences of two or three millimeters read as large visual differences.
The more useful version is relative. Clinicians compare philtrum height against the height of the chin section below the lower lip, with the philtrum at roughly half that height or less as the reference. A philtrum reads long or short relative to the lower third it sits in, so the ratio carries more information than the millimeter figure on its own.
Measuring it from a photo
Straight-on photo, camera at eye height, head level, lips closed and completely relaxed. Any smile pulls the upper lip upward and shortens the apparent philtrum, and pressing the lips together rolls the vermilion border inward and does the same. A relaxed, slightly parted mouth is more reliable than a deliberately closed one.
Because the distance is short, lens distortion from a close phone camera has an outsized effect. Move back and zoom rather than bringing the camera to your face. For a millimeter figure you need a scale reference in frame, such as a card held against the chin; for the ratio against the chin section, pixels are enough.
How much is too much, and what it costs
Inside roughly 11 to 15 mm, and at or under about half the chin height, philtrum length is not something other people notice. Past about 18 mm, or once the philtrum exceeds about half the chin section, the upper lip reads long. Below about 10 mm the upper lip reads short and the upper teeth often show at rest, which is its own visible sign.
The cost of a long philtrum is age. A long upper lip paired with a thin vermilion is one of the most consistent signals observers use to read a face as older, and it is one of the few facial measurements where a change genuinely registers across a room. It also hides the upper teeth when you speak, which flattens the smile. That is why lip lifts are common in people who measure past 18 mm.
What actually changes it
Age lengthens it. The upper lip descends and the vermilion thins over decades as skin loses elasticity, so a philtrum that measured 13 mm at twenty will usually measure longer at fifty with no intervening event. Lip exercises and massage do not shorten it. Nothing done at home moves this measurement.
The procedures that change it are filler, which adds volume to the vermilion so visible lip occupies more of the span without shortening the skin, and a lip lift, which surgically removes a strip of skin under the nose and shortens it by several millimeters permanently.
