Draw a line from the inner corner of your eye to the outer corner and measure it against horizontal. Most adults sit a few degrees positive, meaning the outer corner is slightly higher. Level is neutral. Below level is negative. The tilt is set by the bone of the eye socket and by the tendon that anchors the outer corner of the lids.
Positive tilt is often described as alert or youthful and negative tilt as tired or sad, which is why the term spread in looksmaxxing communities. The effect is real but small at small angles: a tilt just below level is common, and on its own it does not decide how a face is perceived.
How to check it without fooling yourself
Head tilt is the most common reason people believe they have negative tilt. Rotating your head even slightly lowers one outer corner in the photo. That is the range where people start to feel it defines their face.
Tilt that has developed over months or years, especially with the lower lid pulling away from the eye, watering or irritation, is usually lid laxity rather than your natural shape. That is worth showing to an eye doctor, because loose lower lids can affect how the eye is protected, not only how it looks.
What actually changes it
The only structural change is surgery: canthoplasty or canthopexy, performed by an oculoplastic surgeon, which repositions or tightens the outer corner. Results can be good, but risks include dry eye, visible scarring, an unnatural almond shape and asymmetry, and revision is difficult. Thread lifts sold as fox eye procedures give temporary change with their own risks.
Mewing, eye exercises, taping and massage do not move bone or tendon.
